Join our team as a Community Rehabilitation Occupational Therapist and play a key role in improving the lives of adults in their own homes and community settings. In this role, you will:

• Perform comprehensive functional and environmental assessments• Create and implement personalised rehabilitation and care plans• Deliver evidence-based interventions to optimise patient safety, function, and independence• Assess for and recommend adaptive equipment and minor adaptations• Work collaboratively with multidisciplinary teams to provide holistic care• Maintain accurate and timely patient records in line with professional standards

To be successful in this role, you will need:

? A current and valid HCPC registration? UK experience working as an Occupational Therapist in community rehabilitation settings? Strong communication and interpersonal skills? Ability to work independently and within a multidisciplinary team? Commitment to delivering high-quality, person-centred care? Full UK driving licence and access to a vehicle for community visits? This role offers a flexible working position, requiring availability Monday to Friday, with part-time options considered. The contract is ongoing with a strong possibility of extension based on performance and service needs.
