We are seeking an experienced Care Coordinator to join our dedicated team on a locum basis, supporting our domiciliary care and supported living services across Nottingham.
This is a key role, coordinating care delivery, managing staff rotas, and ensuring that every service user receives the highest standard of person-centred care.
Key Responsibilities:
Allocate and schedule care staff based on client needs
Monitor care delivery and complete spot checks and audits
Update care plans, risk assessments, and documentation
Support with onboarding and staff performance monitoring
Liaise with service users, families, and professionals to ensure satisfaction
Requirements:
Previous experience in care coordination or scheduling
Knowledge of CQC standards and person-centred care
Strong organisational and communication skills
Driving licence and vehicle preferred
