Care Coordinator (Regional Care Lead)
Location: Chesterfield, Head Office
Hours: Full-time, 37.5 hours per week (MondayFriday, 9am5pm)
Salary: £28,000 £35,000 per annum (depending on experience)

For this position you will receive on-going and enhanced training in complex Autism and associated mental health issues to meet the specific needs of each individual person you will be supporting.

You will be part of an exciting HCD Genesis project and of a new specialist team to bring people, with complex Autism and associated mental health issues, out of secure units and take them back home to their local community.

About the Role

You will be part of an initial team of two experienced Care Coordinators (Regional Care Leads) to work at our head office in Chesterfield, to support individual people with complex autism and associated mental health issues, being discharged from secure units in the England. You will be supporting our RMN nurse team covering the whole of England and Wales and play a key role in this exciting new project to bring people out of secure units and return them home to their local community.

Key Responsibilities

  • Coordinate and attend team client reviews
  • Act as the key point of contact for clients, staff, families and stakeholders
  • Lead on organising training, appraisals and supervision for care staff
  • Maintain detailed and accurate records
  • Attend essential training, including some off-site sessions

What We're Looking For

  • Level 3 Health & Social Care qualification (or equivalent)
  • Strong background in care coordination, ideally in complex care
  • Specific experience working with complex autism and associated mental health issues
  • Excellent communication, organisation and record-keeping skills
  • Sound understanding of CQC compliance
  • Ability to manage and motivate teams
  • Confident using Microsoft Office (Word, Excel, Outlook)
  • Full UK driving licence and own vehicle
