Location: Chichester
We are seeking a dedicated Clinical Pharmacist to join a well-established practice in Chichester. You will play a key role in optimising medication management and improving patient outcomes.
Key Responsibilities:
<ul>
<li>Conduct medication reviews and provide expert advice on prescribing.</li>
<li>Work closely with GPs and other healthcare professionals to ensure safe and effective patient care.</li>
<li>Support patients with long-term conditions by optimising their medication regimes.</li>
<li>Conduct audits and implement strategies for improved prescribing practices.</li>
</ul>
Requirements:
<ul>
<li>Registered with the General Pharmaceutical Council (GPhC).</li>
<li>Experience in a primary care or clinical setting preferred.</li>
<li>Strong knowledge of medicines optimisation and prescribing protocols.</li>
<li>Excellent communication and problem-solving skills.</li>
