We are looking for someone based in the South of England ideally around the Reading area as this is central to the clients you will cover.

Our Head Office is based in Leyburn, North Yorkshire, however this is a fully remote role and you will be home based and not expected to travel to the office.

Were looking for a confident and capable Regional Care-Coordinator to conduct client assessments, bi-monthly carer spot checks, in-the-moment training and general administration for clients and carers across the South and South East of England. This is a flexible, remote role ideal for someone who takes pride in high standards, and thrives on the road. Occasional overnight stays may be required.

The Role

As Regional Care-Coordinator, youll:

  • Conduct initial assessments with new clients:

  • Build strong, reassuring relationships with clients and families

  • Write personalised care plans and risk assessments using our tailored platform

  • Liaise to ensure smooth, professional service setup

Review care plans every six months or when care needs change

Conduct bi-monthly carer spot checks:

  • Assess new carers' competency

  • Provide in-the-moment coaching or training

  • Ensure care quality aligns with expectations and training

  • Identify and escalate any concerns or further training needs

Document clearly and professionally, submitting daily reports via our digital systems

Be a reliable point of contact for clients, carers, and the office

Use initiative and discretion to handle day-to-day care issues calmly and effectively

Support our on-call service on a rota basis

What Youll Need

Essential:

  • Sound understanding of CQC regulations and care quality standards

  • Experience as a Care Coordinator, Healthcare Professional, or Care based Trainer

  • Strong customer-focused approach and confident, polished manner

  • Ability to think on your feet use initiative and work independently

  • Excellent communication and rapport-building skills, especially with clients

  • Confident using digital tools for documentation and report writing

  • A genuine enjoyment of being on the road and meeting people

  • Full UK driving licence and own vehicle

  • Willingness to travel regularly across the South / South East (occasional wider travel as needed)

Desirable:

  • NVQ Level 3 (or equivalent) in Health & Social Care

  • Registered Manager experience
