Holes Lane Medical Centre is a progressive, innovative, adaptive, and high achieving training practice established in Woolston, Warrington. We have 2 branches located within easy reach of the M62 and M6 motorways that serve a population of 10,200 patients.

In April 2024, we introduced My Care. A new approach in the management of care for patients with long-term conditions. It is collaborative, empowering the patient to have a quality of life by addressing their needs holistically and promoting shared decision making with the support of the clinician. Feedback from patients and staff has been very good. Continual nurse training will be offered to keep this initiative a success.
Job Description:
We seek an enthusiastic and experienced Senior Practice Nurse to join our dynamic team. This motivated and proactive individual must possess the relevant skills in Diabetes management and ideally hold prescribing qualifications. They will be expected to engage fully with the processes that maintain high QoF performance and deliver evidence-based clinical care. This individual would be expected to uphold the values of the practice described above.
Key Responsibilities:
- Understand and the management of QOF requirements, which include monitoring of all patients with long-term conditions. With a special focus on diabetes care (previous experience essential)
- Engaging public health campaigns
- Mentoring
- Infection Control Lead
- Attend regular meetings between the nursing and wider clinical team.
- Attend regular education programmes.
- Ensuring annual registration and training compliance is always adhered to.
Qualifications and Skills:
Essential
- Registered Nurse with a current NMC registration and practicing certificate.
- Nurse Prescribing Qualification is essential.
· Proficient in diabetes management, with strong knowledge of current best practices.
· Excellent communication and teamworking skills.
· Proficiency with electronic patient record systems, ideally Systemone (TPP).
· Competence with Microsoft Word and Excel.
· Commitment to continuous professional development and evidence-based practice.
Desirable
