As a Care Coordinator, your responsibilities will include: Coordinating and overseeing the mobilisation of new complex care packages
Managing the process of new package set-up including care plans, risk assessments, and compliance with regulatory requirements
Developing relationships across the business to ensure the bespoke service runs seamlessly
Liaising with key stakeholders including clients, families, healthcare professionals, and internal teams to ensure a smooth and timely delivery of care

The ideal Care Coordinator candidate will have:
Previous experience in care package coordination
Proven leadership skills
Knowledge of the legislation affecting health and social care
Excellent communication skills, both verbally and in writing
The ability to gain the trust, commitment and cooperation of others
Strong organisational skills
A flexible and creative problem-solving ability
Numeracy skills and the ability to analyse complex issues, absorb information and understand data
