Trainee Dental Nurse

Care Coordinator - £30,000 - £34,000 based in Wandsworth The successful candidate will be responsible for a geographical area, owning all the client relationships in that area, managing their clients care requirements by liaising with their GP’s, District Nurses, OT’s Discharge, pharmacist, family members etc, as well as their team of Care givers and supervisors, and will be required, at a minimum to template and maintain a 4-week call roster. Duties

*Main Responsibilities* * Monitor all information on CRM to ensure that you are aware of all New Business Meetings in the Region each day.
* Update CRM and the Resourcing Report with the status of all Meetings and their status/progress.
* Liaise with Clients and their families to ensure that we communicate efficiently and effectively how and who will be caring for themselves or their families. Take accountability and “own” the relationship with the client and their families and other stakeholders until the commencement of care and ensure the smooth transition to the relevant LIRM and CC who will be overseeing the client moving forward.
