Evidence of supporting vulnerable individuals through a change process
Experience of caseload management demonstrated via the use of a Case Management System

About the Role
Being an Occupational Therapist in our Surrey Heath Locality Team will facilitate your development and equip you with a range of skills to enable you to undertake frontline holistic occupational therapy practice within a social care setting.
You will work alongside a variety of clients who have differing and/or complex needs by using a strength-based approach. Clients may be elderly and frail, experience a learning disability, physical or sensory impairment, or have mental ill health. This diverse role will facilitate your development and equip you with a large range of skills to enable you to undertake frontline holistic occupational therapy practice within a social care setting. You will have the opportunity to put into practice assessments, such as the moving and handling risk assessments and intervention, with the aim of supporting and empowering our clients to live meaningful lives within the community.
You will work closely with your team members, as well as with other organisations and agencies, to provide an integrated service in order to support vulnerable adults and their carers in a community-based setting. This community-based role requires you to visit clients in their homes, as well as other settings such as residential care and nursing homes.

Responsible for caseload of referred patients with a range of conditions/needs.
Undertake comprehensive functional assessments of patients using knowledge of evidence-based practice, and rehabilitation expertise.
Monitor and reassess treatment and patient status to progress and adjust treatments effectively.
Develop comprehensive discharge plans, referring to specialist services as indicated.
Responsible for the competent use of equipment and patient appliances as indicated. About You HCPC registered with a minimum, of 3 years experience.
Ability to provide physiotherapy assessments and treatments to patients in their own homes.
Documented evidence of continuing professional development.
Knowledge of the principles of clinical governance and its implications for occupational therapy.
Awareness of assessing/ managing/preventing risk in the working environment.

Role overview:
As a disability Assessor, you will draw on your experience as a clinical expert to assess and examine individuals with disabilities and illnesses who have applied for Personal Independence Payment (PIP). You'll listen sensitively to their experiences, ask questions and use your insights to report on how their disability or health condition is affecting their daily life. Your factual report will help the Department for Work and Pensions (DWP) determine their eligibility for benefits.
You will be supported in managing your caseloads efficiently, with the time to give consultations the care and attention they deserve. You will not be expected to work long days, do night shifts or take work home with you! We are currently undertaking telephone assessments with claimants.

Requirements:
Currently and fully HCPC or NMC registered with a valid licence to practise in the UK.
Eligibility to live and work in the UK (no sponsorship opportunities are available).
Up-to-date evidenced CPD portfolio and appraisal.
Government legislation requires at least 1-year post-qualification work experience.
Previous experience as an assessor is not required as you will be fully trained to do the role.
A UK driving licence is NOT required for these roles.
