As a care co-ordinator, you will be responsible for:• Managing and co-ordinating the care packages of our clients, ensuring that they receive the right care at the right time from the right carer• Liaising with clients, families, carers, health and social care professionals and other agencies to ensure effective communication and collaboration• Monitoring and reviewing the quality and outcomes of the care delivered, and identifying and resolving any issues or concerns• Supervising care staff, and ensuring that they comply with the policies and procedures of the organisation and the relevant regulations and standards• Maintaining accurate and up-to-date records and reports using our electronic care management system• Participating in the on-call rota and providing support and guidance to care staff and clients as needed

Daytime Healthcare are working with an established Home Care Provider, who are expanding and looking for an Care Coordinator to join their friendly team! You could have the opportunity to work alongside a National Care provider! Duties Include: * The successful candidate will coordinate the delivery of a safe and well-planned domiciliary care service that is service user focused * Responsible for carrying out initial assessments, doing reviews and spot checks * Creating service user care plans, peform assessments and reviews * Planning staff rota's using the bespoke system * Particiape in the branch on call rota * Supporting the new and existing care staff team and coordinating the key carers in your designated areas * Carry out staff supervision and appraisals * Ensuring that all operations are Company and CQC Compliant Requirements: * Good organisational and communication skills to ensure an excellent service * A minimum of 1 year Care Coordinator experience in domiciallry care * Experience doing rotas and care plans * Full drivers licence and access to vehicle is essential * Good IT skills * Ability to work within a fast-paced enviroment to deadlines Schedule: * Full time, 40 hours per week, 9am - 5pm * On call on a rota basis Apply Now to secure a quick interview...
