The successful applicant will form part of the Integrated Network Service and take a lead role in work linked to the Integrated Complex Dementia Day Assessment Service. They will undertake therapy support plans and provide a full occupational therapy function around the needs of the person living with a dementia. This may occur at day centre; in someone’s home or within the community. The worker will focus and well-being and independence goals. They will have a specific focus on hospital avoidance; strength based practice and working to the aims of the Dementia Pathway of Standards in Wales
