We are looking for a therapist with excellent communication skills who can demonstrate sound clinical assessment skills who has the ability to work autonomously as well as part of a team. The role will involve a holistic approach to encompassing physical rehabilitation and refer to wider community services.
You will improve experience for patients and their families. We promote continuity of care for patients in the community setting. Supporting patients following falls and providing support for the Integrated Neighbourhood Team (INT).

Care Coordinator - £30,000 - £34,000 based in Wandsworth The successful candidate will be responsible for a geographical area, owning all the client relationships in that area, managing their clients care requirements by liaising with their GP’s, District Nurses, OT’s Discharge, pharmacist, family members etc, as well as their team of Care givers and supervisors, and will be required, at a minimum to template and maintain a 4-week call roster.
