We are looking for a highly skilled and motivated Occupational Therapist to focus on work with adults living with dementia and people who support them. The successful applicant will form part of the Integrated Network Service and take a lead role in work linked to the Integrated Complex Dementia Day Assessment Service. They will undertake therapy support plans and provide a full occupational therapy function around the needs of the person living with a dementia. This may occur at day centre; in someone’s home or within the community. The worker will focus and well-being and independence goals. They will have a specific focus on hospital avoidance; strength based practice and working to the aims of the Dementia Pathway of Standards in Wales
