Job description

We are seeking an experienced Clinical Pharmacist who is an Independent Prescriber to support a service in Preston. This position requires an individual who is confident working autonomously and has current clearances related to drug misuse services and secure environments. The successful candidate will play a key role in delivering high quality medicines management, prescribing support and patient-centred care within a busy clinical setting.

Key Responsibilities:

  • Provide clinical pharmacist support including medication reviews and medicines optimisation
  • Prescribe safely and appropriately as an Independent Prescriber
  • Manage patients with long term conditions and complex medication regimes
  • Work closely with multidisciplinary teams to ensure effective clinical pathways
  • Maintain accurate prescribing and clinical documentation

Essential Requirements:

  • Fully qualified Clinical Pharmacist
  • Independent Prescriber (IP) qualification
  • Drug misuse clearance
  • Prison clearance or eligibility to obtain it
  • Strong clinical decision making and communication skills
