Job description
We are seeking an experienced Clinical Pharmacist who is an Independent Prescriber to support a service in Preston. This position requires an individual who is confident working autonomously and has current clearances related to drug misuse services and secure environments. The successful candidate will play a key role in delivering high quality medicines management, prescribing support and patient-centred care within a busy clinical setting.
Key Responsibilities:
- Provide clinical pharmacist support including medication reviews and medicines optimisation
- Prescribe safely and appropriately as an Independent Prescriber
- Manage patients with long term conditions and complex medication regimes
- Work closely with multidisciplinary teams to ensure effective clinical pathways
- Maintain accurate prescribing and clinical documentation
Essential Requirements:
- Fully qualified Clinical Pharmacist
- Independent Prescriber (IP) qualification
- Drug misuse clearance
- Prison clearance or eligibility to obtain it
- Strong clinical decision making and communication skills
